  • These general terms and conditions also apply to any additional agreements, treatment agreements and services to the patient.
  • The code of conduct and professional rules of the Royal Society of Physiotherapy (KNGF) are part of the agreement. The patient is aware that the practitioner must respect the rules of conduct imposed by the Royal Association of Physiotherapy (KNGF) when carrying out his practice. The patient accepts the consequences that may result from this

  • Physio Motivati ​​is entitled, if in its opinion there is a need to do so, to change and/or supplement these general terms and conditions.

2. Privacy

3. Data

3. Payment conditions
These payment conditions are inextricably linked to the treatment agreement between you as a patient and the physiotherapist.

Our practice has concluded contracts with almost all health insurers, which means that the practice almost always submits invoices directly to the health insurer and no invoice is sent to the client. Bills that are refused by the insurance company because, for example, the coverage has been exceeded, are submitted to the client. The client is responsible for being aware of his/her reimbursement for physiotherapy. Do you have doubts about your compensation? Ask your health insurer or your physiotherapist.

The client is responsible under all circumstances for full payment for the services rendered to the client or on behalf of the client.

4. Absence
Make sure you are present at the agreed time for your treatment. If you are unable to attend, you must report this at least 24 hours in advance. We are forced to charge for appointments that are not canceled within 24 hours.

Payment for the physiotherapy services is made within 30 days of the invoice date. After this period has expired, the physiotherapist is free to take collection measures without further notice of default being required.

All costs involved in the collection of invoiced amounts, including the extrajudicial costs, will be borne by the client.

5. Complaints procedure
The KNGF has set up a complaints procedure within the framework of the Right to Complaints Act for Healthcare Sector Clients. If you have a complaint, you should first discuss it with the relevant physiotherapist. You should look for a solution together. If this does not work to your satisfaction, you can submit your complaint in writing to: Centraal Bureau Physiotherapy Attn. 6. Quality
The practice complies with legal obligations with regard to quality. The Practitioner within our practice is registered in the Central Quality Register of the Royal Association of Physiotherapy (KNGF) and in the BIG.-register. Within our practice there is a continuous independent quality investigation into the care we provide, which is carried out by Qualiview. When the treatment is terminated, the patient (if he has given permission) will receive an invitation by e-mail to assess us. This data is stored anonymously. This data is inventoried for the following reasons:

  • For ourselves; to improve our practice and care in this way
  • For the health insurer; this will use your review to conclude new contracts with us.

7. Confidentiality
The physiotherapist has the duty to remain silent with regard to any secret that he/she knows or should reasonably suspect that he/she is obliged to keep by virtue of his/her task or profession (art.16 duty of confidentiality).

8. Liability

  • If you leave a coat or other belongings in the waiting room and/or rehearsal room, make sure that there are no valuables in it.
  • The practice is not responsible or liable for damage, loss or theft of property in the building or in the immediate vicinity of this building.
  • Physio Motivati ​​is not liable for damage, of whatever nature, caused by the incorrect and/or incomplete information provided by the patient, unless this inaccuracy or incompleteness should have been known to the practice.
  • Physio Motivati ​​is not liable for damage resulting from a suspension of treatments and/or services for the patient, if that suspension is the result of the patient’s failure to pay the invoices from the practice on time.
  • Third parties cannot derive any rights from the content of the treatments and/or services provided. The patient indemnifies Fysio Motivati ​​against claims from third parties who claim to have suffered damage due to or in connection with treatments and/or services provided by Fysio Motivati ​​for the patient.
  • Fysio Motivati ​​is not liable for any shortcomings in treatments and/or services provided by third parties with regard to the agreement.
  • Incidentally, any liability of Fysio Motivati ​​for damage arising from or related to an attributable shortcoming or tort, or that is based on any other legal ground, is limited to the amount paid or payable by the professional liability insurer of Fysio Motivati ​​in this respect. amount, increased by its deductible under that insurance, unless there is intent or gross negligence.
  • Limitations of liability in favor of Fysio Motivati ​​also extend to the benefit of employees and observers of Fysio Motivati.
  • All claims of the patient lapse twelve months after the moment at which the person concerned became aware or could reasonably have been aware of the existence of these rights of claim or other rights or powers.
